Need help identifying a monitor

I just got this monitor that needs a cap kit and possibly some more work but I cant find the model of it. It is in a wells gardner chasis and it is a 25" monitor that is a CRT flat screen. The unit is a VGA monitor. I will attach some pics that I took via my ipad (not the best quality), any help would be great!
